PERSONAL DATA PROTECTION LAW APPLICATION FORM

GENERAL EXPLANATIONS

Under the Law on the Protection of Personal Data No. 6698 ("PDPL"), personal data owners who are defined as the data subjects (hereinafter referred to as the "Applicant") are granted certain rights related to the processing of their personal data under Article 1 of the Personal Data Protection Law.

According to Article 13, Paragraph 1 of the Personal Data Protection Law, applications regarding these rights must be submitted to our Company ("Data Controller") in writing or through other methods that may be determined by the Personal Data Protection Authority ("Authority").

In this context, applications to our Company in writing may be made by printing this form and submitting it by:

• Personal application of the Applicant,

 

• Via Notary,

1. Written Application	Personal application with a wet signature or via Notary	Hadımköy Mahallesi İbni Sina Caddesi No:8/5 ARNAVUTKÖY/İSTANBUL	The envelope/notification must state "Personal Data Protection Law Information Request".
2. Registered Electronic Mail (KEP) address	Via Registered Electronic Mail (KEP) address	[email protected]	The subject of the email must state "Personal Data Protection Law Information Request".
3. Application with Email Address Registered in the Company System	Using the email address registered in Çağdaş Kağıt Ambalaj San. ve Tic. Ltd. Şti. system	[email protected]	The subject of the email must state "Personal Data Protection Law Information Request".
4. Application with an Email Address Not Registered in the Company System	Using an email address not registered in the Çağdaş Kağıt Ambalaj San. ve Tic. Ltd. Şti. system, with mobile signature/e-signature	[email protected]	The subject of the email must state "Personal Data Protection Law Information Request".

The Applicant may send the application to us via the secure electronic signature defined in the Electronic Signature Law No. 5070, by signing with a "secure electronic signature" and sending it to the Company's registered electronic mail address.

 

Applications sent to us will be answered within thirty days from the date the request is received, in accordance with the provisions of Article 13, Paragraph 2 of the Personal Data Protection Law, depending on the nature of the request. Our responses will be provided in writing or electronically in accordance with the relevant provision of the PDPL.
